Sick certificates

As per government guidelines, all patients MUST complete a 7 days self-certification before requesting a medical certificate from their GP.

You can self-certify if you’ve been ill for less than a week (up to 7 days) using this form:

You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days. Your employer can also provide you with an SC2 self-certificate form for shorter periods of illness.
